Fees And Dues:

The Membership begins with this agreement. The total membership fee consist of 1) Initiation Fee 2) Monthly Membership Dues 3) Restaurant Fee (Credit) 4) Annual Membership Fee.

1) INITIATION FEE:
Member agrees to pay a one-time initiation fee to qualify for membership. In the around of $ 000.00 . Member and club agree that the initiation fee is a one-time non-refundable fee. All membership are non-proprietary and non-voting.

2) MONTHLY MEMBERSHIP DUES:
Monthly due for this  -__________ type  of membership are $000.00 payable one the first day of each month, payable upon signing of this agreement.

3) RESTAURANT FEE (Credit) :  $10.00 per month restaurant fee (entities member to $10.00 credit in restaurant purchases. )

4) ANNUAL MAINTENANCE FEE is payable with this agreement and may be prorated on predetermined scale. The entire Annual Maintenance Fee is payable annually on January 1.

TERM: Initial term begins on ______ and ends on ________. Agreement will continue to renew on a monthly basis until cancelled by the member or club.

If written notice of termination is given on OR before the 15th day of month. termination will be effective the last day of current month.
